چکیده

In a retrospective study by a Canadian team [1], pentastarch infusion was a dose-related independent risk factor for acute kidney injury (AKI) after cardiac surgery. In a new retrospective cardiac surgery study by that team [2], 83% of patients received hydroxyethyl starch (HES) 130/ 0.4. For unexplained reasons, 25 to 43% of patients received both HES 130/0.4 and pentastarch. The team 'hypothesized that both synthetic starches and albumin-containing solutions are independently associated with AKI following cardiac surgery in a dose-dependent fashion'. However, they focused on albumin and never thoroughly evaluated HES-related AKI. Although univariate analyses were reported, propensity matching according to either HES 130/0.4 or pentastarch administration was omitted. Systematic allocation of low-risk patients to HES could have masked an association with AKI in the univariate ana-

Nephrotoxicity of hydroxyethyl starch 130/0.4 infusion in patients receiving coronary artery bypass surgery: a retrospective study

Background: Hydroxyethyl starch (HES) is helpful to maintain the stable haemodynamics in patients receiving cardiac surgery. However, there are concerns regarding the nephrotoxicity of HES. Objectives: This study was to evaluate the nephrotoxicity of HES 130/0.4 in patients receiving on-pump coronary artery bypass surgery. Methods: Patients receiving on-pump coronary artery bypass surgery in Xi...

Mixed colloids and acute kidney injury: a case of selection bias?

In a previous issue of Critical Care, Frenette and colleagues [1] described a retrospective study of 984 cardiac surgery patients receiving mixed colloids, including hydroxyethyl starch (HES) 130/0.4 in 82% or HES 200/ 0.5 in 43%. In 25% or more of the patients, both HES solutions were used. A small minority (16%) received 5% or 25% albumin. An association was observed between albumin exposure ...

Renal effects of synthetic colloids and crystalloids in patients with severe sepsis: a prospective sequential comparison.

OBJECTIVES Hydroxyethyl starch 200 is associated with renal impairment in sepsis, but hydroxyethyl starch 130/0.4 and gelatin are considered to be less harmful. We hypothesized that fluid therapy with only crystalloids would decrease the incidence of acute kidney injury. DESIGN Prospective sequential comparison during intensive care unit stay. SETTING Surgical intensive care unit. PATIENT...

Incidence of postoperative death and acute kidney injury associated with i.v. 6% hydroxyethyl starch use: systematic review and meta-analysis.

BACKGROUND Trials suggest that the use of i.v. hydroxyethyl starch (HES) solutions is associ-ated with increased risk of death and acute kidney injury (AKI) in critically ill patients. It is uncertain whether similar adverse effects occur in surgical patients. METHODS Systematic review and meta-analysis of trials in which patients were randomly allocated to 6% HES solutions or alternative i.v...

The effect of hydroxyethyl starches and crystalloid fluids on mortality and need for dialysis in vascular surgery patients

Introduction Previous research has indicated that fluid choice is an important component of the resuscitation of the critically-ill patient. Synthetic hydroxyethyl starches (HES) may remain within the intravascular space compared to crystalloid solutions, yet they have also been associated with acute kidney injury. Data comparing HES and crystalloid solutions in anesthesia is incomplete. The go...
